Supported Independent Living Program (SILAP) helps transition clients from ICF group homes onto waivers

SILAP is working with clients entering or returning into the workforce. A SILAP advocate assisted a new client transition to a new provider after several issues with staffing in her home. SILAP has been working with transitioning clients from ICF group homes onto waiver by assisting in team meetings, service monitoring and adapting to their new environment in the community on Medicaid waivers.
